• Direct Credit Transfer Request - Complete this form to provide the relevant organisation with your new Direct Credit account details.
• Direct Debit Transfer Request - Complete this form to provide your relevant service provider with your new Direct Debit account details.

You can list your regular payments on this form to advise relevant organisations your new bank account details via fax or mail. You can fax the forms to 1300 132 755. BankSA will forward these forms to the relevant organisations on your behalf within 2 business days.
